Providence Family Medicine Center earns national recognition for patient-centered care

 

Care model improves quality and efficiency of care
 
ANCHORAGE, AK – Providence Family Medicine Center has been awarded Recognition as a Level 3 Physician Practice Connections® Patient-Centered Medical Home™ by the National Committee for Quality Assurance. PFMC is only the third clinical organization in Alaska to receive Recognition.
 
The Patient-Centered Medical Home is a model of health care delivery that aims to improve the quality and efficiency of care. The standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.
 
“For patients, this means they are more empowered to manage their health,” says Harold Johnston, MD, director of physician programs for Providence Family Medicine Center. “They partner with their care team in making decisions about reaching health care goals.”
 
The Recognition Programs are built on evidence-based, nationally recognized clinical standards of care. To receive Recognition, which is valid for two years, Providence Family Medicine Center demonstrated the ability to meet key program components in the following areas: 
  • Written standards for patient access and enhanced communications
  • Appropriate use of charting tools to track patients and organize clinical information
  • Responsive care management techniques with an emphasis on preventive care
  • Adaptation to patients’ cultural and linguistic needs
  • Use of information technology for prescriptions and care management
  • Use of evidence-based guidelines to treat chronic conditions
  • Systematic tracking of referrals and test results
  • Measurement and reporting of clinical and service performance
Providence Family Medicine Center, part of Providence Health & Services Alaska, offers a full range of primary care services for families, including acute and chronic disease treatment, preventative care, minor surgery, pediatrics and geriatrics. It also hosts the Alaska Family Medicine Residency program, which is designed to train physicians to practice in both urban and rural settings in Alaska.
